The CVC Program provides proactive care coordination for Veteran Gold Card holders with chronic health conditions and Veteran White Card holders with chronic DVA-accepted mental health conditions. 

Our Care co-ordinator (Practice Nurse) will be working with your doctor to ensure your care to optimise your health and wellbeing to reduce the risk of frequent hospital admissions.
