Appointments :
If you would like to schedule an appointment with your doctor, please contact reception on 03 4327 0755 or you can make a quick and easy appointment through our ” Book Now ” link. Please note many of the doctors consulting at JSN Medical also welcome all walk-in patients. The friendly staff at reception will always try to find a suitable appointment time for you with your doctor. Please be aware that emergencies will be given priority by the doctors operating on the practice. If this situation should occur when you are scheduled to attend the practice and there is an extended delay, our receptionists will make every effort to contact you and make alternative arrangements.
Cancellation Policy :
- Cancellation without at least 4 hours prior notice will incur the cancellation charges for the lost time.
- To cancel the appointment, you must get the confirmation of the cancellation via phone / email / text messages.
Walk-in patient will be attended to as soon as possible, although patients with an appointment or emergencies will be given priority.
Non-English Speaking and the Hearing Impaired :
Whilst the Doctors at this Practice are multi-lingual (English, Punjabi, Urdu, Hindi)we understand that there are patients who may require an interpreter. Our Practice can request telephone interpreter for your convenience. Please let the reception know your requirement of interpretation services so that they can arrange it prior to your appointment.
Immunizations :
The Doctors and Practice Nurse can perform all Immunizations from Infant to Adult at our Practice. All Childhood Immunizations are sent to the Australian Immunization Register (AIR).The Doctors are also available for consultation regarding all your Travel Vaccinations needs.
Phone Calls :
The Doctors in this practice are unable to take general phone calls unless they are an emergency, this is to avoid and minimize interruptions during consultations. If the call is not urgent, you may leave a message and the Doctor will return your call, if possible. Doctors will always take phone calls if they are from another Doctor, Pharmacy, Specialist, Hospital or any other Health Care Provider.
Test Results :
Test Results will NOT be given over the phone. Reception Staff and Nurses do not hold the qualifications to discuss these results with you. Patient confidentiality must also be maintained, we are also unable to discuss your results with any other family member or next of kin unless signed authority and appropriate paperwork is in place. You may call the practice to see if your test results have arrived. Most tests results take 3-5 days to return to your doctor.
Reminders :
Our Practice is committed to the continuity of care for our Patients. We may, from time-to-time, issue you a reminder letter offering you preventative health services such as immunizations, pap smears, cholesterol and Diabetes checks. Please let us know if you do not wish for this to occur.
Referrals :
The Doctors at this Practice may refer you to services outside the Practice. Such as an X-ray, Ultrasound or to see a Specialist. A phone number with the details will be provided for you to call and make an appointment. At the time, Reception staff will fax referrals (such as to a Hospital Outpatients Department) or call on your behalf if there is an urgency or language barrier.
Personal Health Information and Confidentiality :
Your Medical Information is a confidential document.
Practice Policy is to always ensure and maintain the security and privacy of your records and only available to authorized members of Staff.
Our practice will not disclose Personal Health Information to any third parties without consent, you are at risk of harm or unable to give consent.
Personal Health Information may be disclosed for the following reasons:
· For communicating relevant information with treating doctors.
· Specialists or other allied health professionals.
· for follow-up reminder or recall notices
· Accounting procedures, accreditation.
· Disease notification as required by law.
· Court orders or subpoenas.
Our Practice abides by all mandatory reporting guidelines to the relevant government bodies.
If you have any concerns about your Personal Health Information, please discuss this with your doctor. When needed, you would be required to fill a request form for access and disclosure of your Personal Health Information.
MyMedicare Registration :
REGISTRATION IS NOW OPEN
If you would like to register with MyMedicare and choose our clinic as your preferred medical centre, you can now register online or in person at our clinic.
To register online:
First, you will need to start the registration process in your Medicare Online Account or Express Plus Medicare Mobile app. Find our medical centre in the list of registered GP clinics in MyMedicare before you start. Our medical centre staff will then accept the registration in the MyMedicare system. For an easy guide, you can follow these steps to register. When registering online, if you cannot find our clinic in the list of medical centres that are registered with MyMedicare, please check back later inthe month, as we may still be going through the set-up process in the system.
To register in person:
Alternatively, you can visit our medical centre and fill out a registration form at reception. By signing the form, you are giving consent to our medical centre staff to register you in the MyMedicare program. Our practice staff will then complete the registration in the MyMedicare system for you.
Information you Supply :
If you are a new patient, your doctor will require that you complete a new patient registration form. To ensure optimal patient care, your doctor may also request that you inform them of your cultural background. Please inform reception of any changes to your personal information on subsequent visits, such as change of Medicare card details, phone number, address or emergency contact.
Health Information :
All patients have the right to access their personal health information under the Privacy Amendment (Private Sector) Act 2000. On request for access to personal health information, our practice documents each request and endeavors to assist patients in obtaining access. There may be charges involved in providing copies of all or part of your records. Information may not be available immediately, please allow minimum 14 working days for access time to be arranged or information to be collated and copied. If your request is urgent, directly contact your doctor or our practice manager.
Zero Tolerance Policy :
This medical practice has zero tolerance for abusive and/or threatening behavior towards independent doctors, staff members and/or other patients. This includes shouting, swearing, aggressive actions, threats, inappropriate gestures and name calling. If you are abusive or threatening, you will be required to leave the medical centre and transfer your care to another practice. Refusal to leave will result in the police being called.
